LABORATORY DIAGNOSIS

 

Gram-stain exudate

Polymorphonuclear leukocytes (PMNs) intracellular gram-negative diplococci

Diagnostic 90% male, 60% female

Not useful for pharyngeal infection

Culture

  1. Isolation of organism in selective media (Thayer-Martin) is the gold standard.

Culture detects 80% to 90% cervical, rectal, and pharyngeal infections.

  1. Culture all symptomatic site: Recovery of N. gonorrhoeae is improved by two consecutive endocervical specimens or combina­tion endocervical and anal specimens.

Other tests: antigen detection (gonozyme)

  1. All pregnant patients should be screened for N. gonorrhoeae at ini­tial prenatal visit. The test should be repeated early in the third trimester.

 

Polymerase chain reaction (PCR)—based tests (Gen-probe)—becoming gold standard of testing

Unable to measure antimicrobal susceptibility

 

Ofloxacin, 400 mg PO, bid

Levofloxacin, 500 mg PO, qd
