ENDOMETRIOSIS ASSOCIATED INFERTILITY

 

 

When surgery is indicated, the objective is to excise all visible endometriosis, and then allow the patient to conceive, using assisted reproductive technologies as indicated. Older patients and couples with prolonged infertility should be offered assisted reproductive therapies early.

 

  1. Stages I and II

 

  1. Expectant treatment selectively
  2. Laparoscopic surgery with excision of endometriosis
  3. Menopausal gonadotropin or human chorionic gonadotropin (HCG) and intrauterine insemination
  4. Gamete intrafallopian transfer (GIFT), in vitro fertilization (IVF), and endotracheal tube (ET)

 

Stage III: moderate disease

  1. Laparoscopic surgery with excision of endometriosis
  2. Conservative surgery by laparotomy with preoperative danazol
  3. Menopausal gonadotropin or HCG and intrauterine insemination
  4. GIFT, IVF, and ET

Stage IV: severe and extensive disease

  1. Conservative surgery by laparotomy with preoperative danazol
  2. Laparoscopic surgery selectively
  3. GIFT, IVF, and ET
