 Topamax 25 mg. What is the Preparation?

Topamax is an antiepileptic preparation. It is used to treat the children down to the age of two years old and adults. The active component of Topamax is Topiramate.

 

Description of the effects of this substance is ultra-tough complicated, so you should understand that it intervenes into the processes of pathological nerve conduction. The effectiveness of Topamax depends on its concentration in the blood stream. Treatment with Topamax may be a single agent therapy (when this medicine only is used). For example, it is possible in the case of the non-lesional epilepsy. Also, Topamax can be combined with other drugs. As is the case in the different patients’ states, characterized by convulsionary attacks.

 

Topamax is the original drug of Topiramate, safety and efficacy of which is confirmed by the clinical trials.

 

Topamax 25 mg. The Therapeutic Indications

As a single agent therapy

a)      Epilepsy (including patients with the non-lesional epilepsy) in adults and children older than 2 years old.

b)      Migraine.

c)      Prophylaxis of migraine attacks in adults.

As a part of comprehensive treatment:
a) Partial or generalized tonic-clonic seizures in adults and children over 2 years of age.
b) Seizures, coming from Lennox-Gastaut syndrome in adults and children older than 2 years.

 

Topamax 25 mg. Efficiency and Safety

Topamax is included in the International guidelines for the treatment of epilepsy (ILAE, NICE, AAN, FDA) as one of the best drugs for the initial single agent therapy of the partial and generalized seizures.

Topamax 25 mg is approved for use by U.S. FDA in the initial single agent therapy in adults and children ≥ 10 years old with partial and primary generalized tonic-clonic seizures.

 

ILAE recommends Topamax prescription as one of the drugs of the initial single agent therapy to treat the following types of accesses and epilepsy:

 

–          partial seizures in adults;

–          partial seizures in children;

–          partial seizures in elderly patients;

–          generalized tonic-clonic seizures in adults;

–          generalized tonic-clonic seizures in children (level C recommendation);

–          ephebic Unverricht’s or Lafora’s disease (recommendation level D).

 

In accordance with the recommendations of NICE (National Institute for Clinical Excellence), Topamax 25 mg is approved for use as one of the drugs for the initial single agent therapy of the different types of epileptic seizures (generalized and partial):

 

–          generalized tonic-clonic seizures;

–          partial seizures with the secondary generalization or without it;

–          when different forms of epilepsy: epilepsy with isolated generalized tonic-clonic seizures;

–          focal epilepsy (cryptogenic, symptomatic);

–          severe Unverricht’s or Lafora’s disease of infancy;

–          Lennox-Gastaut syndrome;

AAN recommends Topamax prescription as one of the drugs to treat the initial single agent therapy of the partial and mixed types’ seizures.

 

 

Topamax 25 mg. The Recommendations for Administration

Topamax 25 mg has to be taken per os regardless of meal. Tablets should not be parted. If you have difficulties with the pills’ swallowing, use capsules. Mix the capsules’ contents with a small amount of soft food and swallow immediately without chewing.

Adults, when the single agent therapy: the initial dose is 25 mg 1 time a day (at night) for 1 week, then the dose has to be increased up to 25-50 mg/day (portioned into 2 doses) every 1-2 weeks, while you achieve the desired clinical benefits. The recommended dose is 100 mg/day. The maximum daily dose is 500 mg (for some patients with refractory forms of epilepsy – up to 1000 mg /day).

Children over 2 years: in the first week of treatment – 0.5-1 mg/kg nocte, and the dose has to be increased up to 0.5-1 mg/kg/per day (portioned into 2 doses) 1-2 weeks apart. The recommended dose is 3-6 mg/kg/ day (for children with newly diagnosed partial seizures – up to 500 mg/day).

In association with other anticonvulsants: adults – 25-50 mg nocte for 1 week, then the dose should be increased up to 25-50 mg 1-2 weeks apart. The minimum effective dose is 200 mg/ day, the mean daily dose – 200-400 mg, the maximum dose – 1600 mg.

Children over 2 years: – 1-3 mg/kg/ day nocte for 1 week, then the dose should be increased up to 1-3 mg/kg 1-2 weeks apart. The recommended daily dose is 5-9 mg/kg.

 

 

Topamax 25 mg. The Feedbacks

Our son had convulsive sates and before, but he recovered without medications. But this year, there was this incident. As told the neighbor girl, he had a fall at the disco and his tongue tipped out as with dead. There was police there, they thought that he was overdosed. He nearly died in a police-car. An ambulance took my son to the hospital from a police-station.

No drugs have been found. The diagnosis was very complex: tonic-clonic epileptic seizure. The doctors offered Topamax treatment. After the son initiated treatment with Topamax he did no have either convulsions or seizures!!
